Recently, with the aim of effectively consolidating existing elderly care and medical resources, Huai'an City piloted a "two-in-one" model in Chahe Town, Hongze District, integrating medical and elderly care services. Under this initiative, the Chahe Town Central Health Center has assumed direct management of the in-house public nursing home while administratively overseeing the nearby Chahe Town Nursing Home.
Medical and nursing staff from the Chahe Town Central Health Center work part-time in the nursing homes, providing specialized technical support. A psychiatric expert workstation regularly offers psychological counseling for nursing home residents, and a rehabilitation center remains open year-round at no charge for nursing home residents. Moreover, a multidisciplinary collaboration system has been established, allowing doctors from various departments to jointly review nursing home residents’ cases and develop scientifically sound treatment plans.
With medical personnel joining forces, the nursing homes have strengthened their caregiving capacity and enhanced the quality of daily care. They organize regular free health check-ups for all residents, with comprehensive health records maintained. Their scientifically developed meal plans ensure that residents receive nutritious and balanced diets. When a resident falls ill, onsite medical staff can promptly assess the situation and arrange for immediate hospital transfer, facilitating early detection and treatment. During rehabilitation, residents can benefit from personalized, precision medical services provided by the rehabilitation center.
By breaking down the policy barriers that previously restricted medical insurance reimbursements in grassroots health centers, this model enables the health center to admit ill residents promptly and allows residents to secure timely reimbursement for their treatment costs. Making local health centers the primary choice for hospitalization helps residents avoid issues such as fragmented care, out-of-area treatments, and exorbitant nursing fees, thereby reducing healthcare expenses for economically disadvantaged residents.
